To schedule appointments at either my office in Kirkland or Snohomish
you can call my main number at

(press 1 as soon as you hear the greeting to access my voice mail box)
All message are returned within 24 hours but usually sooner
within 4 hours, except those received on my days off
(Saturdays and Sundays) which are returned the
following business day.

From I-405 North or South
Take the Kirkland Exit to 85th St.
Go West on 85th down the hill, it turns into Central Way
Go Left on Lake Street (also known as Lake Washington Blvd)
Take next left onto Park Lane.
Park Lane Wellness Center is on the right side of the street
just before Main Street.
Parking
There is free two hour street parking on Park Lane and
surrounding streets. Free 4 hour parking is
available in the Kirkland Library garage on the corner
of 3rd street and Kirkland Ave (1 block east of
my office).

Insurance Provider
I am a provider at both locations with most insurance plans including Premera,
Group Health, Uniform, Aetna, Cigna and First Choice. I also accept L & I
and PIP claims. All insurance benefits must be confirmed in advance of your
apppointment. Please note that I am unable to accept group health patients
in Snohomish.
Massage Services, Kirkland- Please note that all services and rates
listed below are for my Kirkland location. At both locations I accept cash and
check. In Kirkland I also accept Visa and Mastercard Debit and Credit. There
is a $25 handling fee on returned checks.
Billed Services - for all insurance services
CPT Code 97124 - $30 per unit (1 unit = 15 minutes)
CPT Code 97140 - $35 per unit (1 unit = 15 minutes)
Massage Rates (for payment at time of service)
30 minutes $55 60 minutes $85 90 minutes $102
(There is an additional $5 discount if you pay by cash or check at time
of service).
Massage Packages
Five 30 minute- $250 Five 60 minute-$400 Five 90 minute-$485
(a $25 savings, must be paid in advance and used within 6 months
from the date of purchase. There is an additional $25 discount if
you pay by cash or check).
Cancellation Policy 24 hours notice is required to change or cancel your
appointment. If you do not provide this notice you will be charged for your
appointment. This charge must be paid before or at your next appointment.
Your insurance company will not pay for your cancellation charges. If you
experience a sudden onset of illness, injury or emergency please provide as
much notice as possible. If you are contagious please reschedule your appoint-
ment.
Gift Certificates Available year round, they are a great way to
share the gift of massage with a friend or loved one.
